Website Privacy Statement
This statement explains how the wolf.dental website handles information. How we protect your health information as a patient is described in our Notice of Privacy Practices.
Information you send us
Our contact form asks for your name, phone number, email address, the reason for your message and the message itself. We use this only to reply to you and to schedule appointments. We do not sell it or use it for advertising.
The form is run by our website host, Netlify, which stores your message and emails it to our office.
Please do not include health details, insurance numbers or other sensitive information in the form or in email. Call us at 508-473-4999 instead. For a dental emergency, please call the office rather than using the form.
Other online services
- Secure Referral Form: dental offices sending us patient information use a secure form run by DentalCareLinks, which has its own privacy terms.
- Patient forms: the forms on our Forms page are PDF files you download, fill in and bring to your visit or give to our office. Please do not email completed forms unless we have arranged a secure way to do so.
- Reviews: our Reviews page shows reviews through a service called Elfsight, which loads content from its own servers.
- Fonts: our pages load fonts from Google Fonts, which receives your browser's standard connection details (such as your IP address).
Cookies and tracking
We do not use advertising or analytics tracking on this website. The services above may set their own cookies under their own privacy policies.
